Diversity, Equity, and Inclusion Statement

As part of our commitment to reconciliation and inclusion, we acknowledge that Thrive Together Mental Wellness Centres operates on the traditional and unceded territories of the Anishinaabe Algonquin Nation in Ottawa and the Haudenosaunee and Anishinaabe Peoples in Kingston. We recognize the enduring presence and contributions of Indigenous Peoples to this land and pledge to honor their histories, cultures, and rights. This acknowledgment serves as a call to action for meaningful collaboration and reconciliation in support of Indigenous communities.

  1. Providing assessment and treatment services (in-person and virtual care)
  2. Billing and payment processing
    Meeting legal and regulatory requirements
  3. Teaching and staff education (with safeguards in place)
  4. Conducting quality improvement and risk management
  5. Informing you of events or opportunities (with your express consent)
  6. Other purposes as permitted or required by law

We collect, use, and disclose only the minimum information necessary. You may withhold or withdraw consent for certain uses at any time by notifying your clinician.

Electronic Health Records & Virtual Care

  • All health information is stored in secure electronic health record systems.
  • Virtual care appointments are conducted through secure platforms.
  • Only authorized staff and clinicians involved in your care have access to your records.


Access to Your Health Records

You have the right to:

  • Access your health records held by us
  • Request corrections if you believe your records are inaccurate or incomplete

A reasonable fee may apply for copies of your health records. To exercise these rights or learn more about our privacy practices, please contact your clinician or our office.

You also have the right to make a complaint to the Information and Privacy Commissioner of Ontario (IPC) if you believe your personal health information has not been handled properly:
